IUI<\/strong><\/span><\/h5>\n

Intrauterine insemination (IUI)\u00a0is a simple form of\u00a0fertility\u00a0treatment that\u00a0involves directly\u00a0placing\u00a0a\u00a0specially\u00a0prepared and\u00a0washed sperm\u00a0inside\u00a0a woman\u2019s womb,\u00a0close to the\u00a0time of ovulation.\u00a0<\/span><\/p>\n

It is\u00a0a\u00a0straightforward,\u00a0non-invasive procedure\u00a0very similar to\u00a0a smear (Pap) test,\u00a0which is usually easy and\u00a0painless and lasts for just a few minutes.\u00a0No sedation or painkillers are required.\u00a0\u00a0<\/span><\/p>\n

The procedure involves\u00a0the insertion of a\u00a0speculum into the vagina in the same as way as having a cervical smear test. A\u00a0thin, soft\u00a0catheter\u00a0(tube)\u00a0is then passed\u00a0through the\u00a0cervix\u00a0into\u00a0the womb.\u00a0A\u00a0properly processed\u00a0sperm sample, containing the\u00a0best quality sperm, is injected\u00a0via\u00a0the catheter.\u00a0\u00a0<\/span><\/p>\n

Optimum\u00a0timing and healthy sperm\u00a0increase\u00a0the chances of IUI success.\u00a0\u00a0<\/span><\/p>\n

Typically, IUI is performed just before\u00a0the\u00a0ovulation.\u00a0This ensures that the sperm\u00a0are\u00a0close to where\u00a0they\u00a0need\u00a0to be, at the most fertile point of the\u00a0menstrual cycle,\u00a0hence\u00a0increasing\u00a0the\u00a0chances of conception.\u00a0<\/span><\/p>\n

The sperm\u00a0used in IUI\u00a0is carefully washed and prepared\u00a0in the laboratory.\u00a0The weaker,\u00a0sluggish or non-moving sperm\u00a0are separated from the stronger, healthier,\u00a0fast-moving\u00a0sperm and toxins are removed.\u00a0<\/span><\/p>\n

IUI can be carried out within a woman\u2019s natural menstrual cycle, without using any fertility drugs\u00a0(n<\/b>atural IUI).<\/b>\u00a0<\/span><\/p>\n

However, fertility drugs may be\u00a0administered\u00a0to stimulate egg production and ovulation, and to prepare\u00a0the uterus to receive embryos\u00a0(stimulated IUI).<\/b>\u00a0<\/span><\/p>\n

If\u00a03 to 6\u00a0consecutive\u00a0cycles\u00a0of\u00a0IUI\u00a0have been performed without being successful,\u00a0then\u00a0review\u00a0of the\u00a0treatment plan\u00a0is needed and\u00a0consideration should be given, moving\u00a0to more invasive\u00a0fertility treatments\u00a0such as\u00a0in vitro fertilisation (<\/b>IVF<\/b>)<\/b>.<\/b>\u00a0<\/span>[\/vc_column_text][vc_empty_space height=”50″ hide_on_mobile=””][\/vc_column][\/vc_row][vc_row][vc_column][vc_separator][vc_tta_accordion active_section=”0″ collapsible_all=”true”][vc_tta_section title=”The IUI process” tab_id=”1591170286608-6a69831f-530c”][vc_column_text]The first step is\u00a0to\u00a0have an initial\u00a0medical\u00a0consultation\u00a0to assess your condition and\u00a0agree your personalised treatment plan,\u00a0which will give you the best chance of success.\u00a0\u00a0<\/span><\/p>\n

Once it has been agreed that IUI is the most suitable treatment for you, the next step would be to decide whether your treatment would be a\u00a0natural<\/b>\u00a0without using any fertility drugs\u00a0or\u00a0a\u00a0stimulated<\/b>\u00a0with use of fertility drugs\u00a0cycle.\u00a0<\/span><\/p>\n

During the process, you will\u00a0need to have\u00a02 or 3\u00a0ultrasound\u00a0scans\u202fand blood tests\u00a0to monitor the growth of your follicles and track ovulation.\u202fDepending on the number\u00a0and size of your follicles,\u00a0the thickness of\u00a0the lining of your womb and the results of the blood tests, the decision will be made regarding the appropriate time for insemination.\u00a0<\/span><\/p>\n

You may be asked to\u00a0carry out testing yourself using\u00a0at\u00a0home\u00a0an ovulation predictor kit to detect the hormone\u00a0\u201cLH\u00a0surge\u201d\u00a0that signals ovulation.\u00a0<\/span><\/p>\n

If required, ovulation will be triggered with an injection of\u00a0the hormone\u00a0human Chorionic\u00a0Gonadotrophin\u00a0(hCG),\u00a0in order to mimic the action of the \u201cLH\u00a0surge\u201d\u00a0and\u00a0stimulate\u00a0the release of the egg.\u00a0The advantage of using\u00a0the trigger injection is timing the release of the egg\u00a0and\u00a0planning\u00a0the insemination.\u00a0\u00a0\u00a0\u00a0<\/span><\/p>\n

The sperm will be\u00a0inserted\u00a024 to 36 hours\u00a0after the ovulation trigger or\u00a0the \u201cLH surge\u201d.\u00a0\u00a0\u00a0<\/span><\/p>\n

Your partner is most welcome to attend with you whilst you are having your scans. If you are having treatment using your partner\u2019s\u00a0sperm, on the day of\u00a0the\u00a0insemination he will be asked to produce a semen sample.\u00a0\u00a0<\/span><\/p>\n

Through out the procedure\u00a0a very strict witnessing protocol is followed, with double witnessing at all levels\u00a0to ensure that\u00a0the\u00a0right sperm is inseminated in\u00a0the right patient.\u00a0<\/span><\/p>\n

Following the procedure, it is\u00a0recommended\u00a0to rest for approximately15-20 minutes before going\u00a0back\u00a0home.\u00a0\u00a0<\/span><\/p>\n

You will\u00a0be\u00a0advised\u00a0to take a pregnancy test two weeks after\u00a0your IUI treatment.\u00a0\u00a0<\/span><\/p>\n

Following a positive pregnancy test an ultrasound scan\u00a0needs\u00a0be\u00a0performed\u00a0approximately 3 weeks later.\u00a0\u00a0<\/span><\/p>\n

If negative you\u00a0will be offered a follow up consultation, to review the situation and plan the way forward.\u00a0<\/span>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”Who is IUI suitable for” tab_id=”1591171278022-9cdd6cea-40d8″][vc_column_text]The idea behind IUI is that couples experiencing\u00a0difficulty to get pregnant can benefit from the exact timing and placement of a more potent\u00a0and healthier\u00a0sperm inside the\u00a0womb.\u00a0\u00a0<\/span><\/p>\n

IUI is\u202fideal for women\u202fwho have a good\u00a0egg\u00a0reserve, open fallopian tubes,\u00a0are under 35\u00a0and wish\u00a0to try a less invasive\u00a0treatment before embarking into\u00a0IVF.\u00a0<\/span><\/p>\n

It is particularly appropriate for single women and same sex couples,\u00a0who need only donor sperm to conceive.\u00a0<\/span><\/p>\n

IUI\u00a0may\u00a0also\u00a0be offered to improve the chance of conception\u00a0in cases when there is a mild male factor problem, with mild reduction in the\u00a0sperm count and motility.\u00a0However, it is not appropriate for cases when\u00a0the\u00a0sperm quality is poor.\u00a0<\/span><\/p>\n

The treatment can also be helpful when there is\u00a0a difficulty\u00a0from the male partner\u00a0to ejaculate during intercourse.\u00a0\u00a0<\/span><\/p>\n

It\u00a0can be of particular benefit\u00a0for women with irregular periods who experience difficulty tracking down the ovulation time and therefore\u00a0synchronizing intercourse with ovulation.\u00a0<\/span><\/p>\n

Treatment with\u00a0IUI\u00a0is not recommended\u00a0in cases where\u00a0the fallopian tubes are\u00a0blocked or\u00a0damaged.\u00a0<\/span><\/p>\n

It is important to note that IUI does not have high success rates\u00a0when compared to IVF. Hence\u00a0it may not be the best option for many patients.\u00a0Each\u00a0individual situation needs\u00a0to be assessed\u00a0to ensure\u00a0whether IUI is the appropriate\u00a0option.\u00a0<\/span>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”Investigations prior to IUI” tab_id=”1591171284543-a455b679-79ea”][vc_column_text]\u00a0The fallopian tubes should be open and healthy and there should be no adhesions present that might prevent an egg from having access to either tube. Therefore\u00a0the patency of the tubes is a prerequisite before proceeding with intrauterine insemination.\u00a0\u00a0<\/span><\/p>\n

The test most commonly used to assess the patency of the tubes is the\u00a0Hysterosalpingogram<\/b>\u00a0<\/b>(HSG)<\/b>,<\/b>\u00a0which is an\u00a0X-ray test involving injecting a dye through the cervix.\u00a0\u00a0<\/span><\/p>\n

Alternatively\u00a0Hysterosalpingo<\/b>-contrast-<\/b>sonography<\/b>\u00a0(<\/b>HyCoSy<\/b>),<\/b>\u00a0a simple and well-tolerated outpatient ultrasound procedure\u00a0can be considered.\u00a0<\/span><\/p>\n

The gold standard method for assessing the patency of the tubes\u00a0is\u00a0a\u00a0laparoscopy and dye test<\/b>.\u00a0However\u00a0this\u00a0is an invasive surgical procedure under anaesthetic,\u00a0hence not favourable in many cases.\u00a0\u00a0<\/span><\/p>\n

A\u00a0baseline\u00a0<\/b>u<\/b>ltrasound scan<\/b>\u00a0to exclude the presence of\u00a0any\u00a0ovarian cysts\u00a0and establish the integrity of the lining of the womb (endometrium) is strongly advised.\u00a0<\/span><\/p>\n

The presence of any benign uterine growths such as polyps or fibroids and\/\u00a0or\u00a0the presence of any\u00a0uterine anomalies such as an endometrial septum needs to be excluded before embarking\u00a0onto a fertility treatment.\u00a0\u00a0<\/span><\/p>\n

If any of the above\u00a0findings are\u00a0present then a decision needs to be made whether a surgical procedure,\u00a0hysteroscopy<\/b>\u00a0or\u00a0laparoscopy<\/b>\u00a0would be\u00a0recommended, before starting an IUI treatment.\u00a0\u00a0<\/span><\/p>\n

What tests\u00a0you need to do?\u00a0\u00a0<\/strong><\/span><\/h6>\n

For you<\/b>, the following\u00a0tests are required:\u00a0\u00a0<\/span><\/p>\n

    \n
  • Anti-Mullerian\u00a0Hormone (AMH)- (within 1 year of treatment)\u00a0<\/span><\/li>\n
  • A full hormone profile done between the 2nd\u00a0\u2013 3rd\u00a0day\u00a0of your period (within 1 year of treatment). That would include the following hormones:\u00a0FSH, LH,\u00a0<\/b>Oestradiol<\/b>\u00a0(E2) and Prolactin<\/b>\u00a0<\/span><\/li>\n
  • Blood group and Rhesus status\u00a0<\/span><\/li>\n
  • Haemoglobin\u00a0electrophoresis,\u00a0to exclude carriers of\u00a0thalasaemia\u00a0or sickle cell disease\u00a0<\/span><\/li>\n
  • Rubella\u00a0IgG, to check for immunity to Rubella\u00a0<\/span><\/li>\n
  • CMV (cytomegalovirus)- only if you are using a sperm donor\u00a0<\/span><\/li>\n
  • FBC (within 2 months of treatment)\u00a0\u00a0<\/span><\/li>\n<\/ul>\n
      \n
    • Thyroid stimulating hormone (TSH) (within 2 months of treatment)\u00a0\u00a0<\/span><\/li>\n
    • Vitamin D (within 2 months of treatment)\u00a0<\/span><\/li>\n
    • Chlamydia test (within 2 years of treatment)\u00a0\u00a0<\/span><\/li>\n
    • Cervical smear\u00a0(Pap- test)- in line with national screening guidelines\u00a0\u00a0<\/span><\/li>\n
    • Mammogram \u2013 if aged 40 years or above \u2013 In view of the increased prevalence of breast disease we would recommend screening prior to commencing a treatment (unless one has been performed within the last 3 years).\u00a0\u00a0<\/span><\/li>\n<\/ul>\n

      Furthermore the following blood tests are\u00a0mandatory<\/b>\u00a0for you and need to be done\u00a0within 6 months<\/b>\u00a0of the treatment:\u00a0<\/span><\/p>\n

        \n
      • HIV\u00a0\u00a0<\/span><\/li>\n
      • Hepatitis B surface antigen (HBsAg)\u00a0\u00a0<\/span><\/li>\n
      • Hepatitis C (Anti-HCV)\u00a0<\/span><\/li>\n
      • Syphilis\u00a0<\/span><\/li>\n<\/ul>\n

        If you have a\u00a0male partner<\/b>\/ husband using his own sperm\u00a0he\u00a0will need to have the following\u00a0tests:\u00a0\u00a0<\/span><\/p>\n

          \n
        • Blood group and Rhesus status\u00a0<\/span><\/li>\n
        • Haemoglobin\u00a0electrophoresis,\u00a0to exclude carriers of\u00a0thalasaemia\u00a0or sickle cell disease\u00a0<\/span><\/li>\n
        • Semen Analysis\u00a0& Sperm culture\u00a0(within 12 months of treatment).\u00a0This\u00a0is essential as poor sperm quality is a contra-indication to this type of fertility treatment.<\/span><\/li>\n<\/ul>\n

          Furthermore the following blood tests are\u00a0mandatory<\/b>\u00a0for him and need to be done\u00a0within 6 months<\/b>\u00a0of the treatment:\u00a0<\/span><\/p>\n

            \n
          • HIV\u00a0\u00a0<\/span><\/li>\n

            \nIUI can be carried out within a woman\u2019s natural menstrual cycle and, unlike\u202fconventional IVF, without the need for intensive medication.\u00a0\u00a0<\/span><\/p>\n

            The lack of fertility drugs and\u00a0invasive\u00a0procedures means that IUI is\u00a0a very safe\u00a0form of\u00a0assisted reproduction treatment, which\u00a0avoids the potential side effects associated\u00a0with some forms of IVF, hence\u00a0making it an excellent option for many women.\u00a0<\/span><\/p>\n

            Non-invasive<\/strong><\/span>
            \nIn IUI there is no egg collection involved, meaning that there is no invasive procedure. Furthermore the procedure is painless and no sedation or anaesthetic is needed.\u00a0<\/span><\/p>\n

            Affordable<\/strong><\/span>
            \nIUI is\u00a0a relatively affordable fertility treatment, and many individuals or couples will\u00a0opt\u00a0to try IUI before moving forward with the more intensive and expensive\u202fIVF\u202fprocess.\u202f\u00a0<\/span><\/p>\n

            Good for single women & lesbian couples<\/strong><\/span>
            \nIUI is\u202fideal for women\u202fwho have a good ovarian reserve and\u202fopen fallopian tubes. Hence, it is often most used by single women and same-sex couples, who need only donor sperm to conceive.\u00a0<\/span><\/p>\n

            Acknowledging\u00a0the benefits of IUI, including the lower cost,\u00a0the\u00a0lower time commitment, and\u00a0the\u00a0lower stress,\u00a0helps\u00a0appreciating\u00a0why many individuals and couples\u00a0consider\u00a0trying\u00a0IUI\u00a0as the first option.\u00a0<\/span>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”IUI risks” tab_id=”1591171286178-5dfbc9ca-cca4″][vc_column_text]IUI\u00a0is\u00a0a\u00a0straightforward,\u00a0non-invasive procedure,\u00a0which is\u00a0generally easy and painless.\u00a0However, occasionally,\u00a0some women may experience temporary, menstrual-like cramping.\u00a0<\/span><\/p>\n

            In a stimulated IUI multiple big size follicles may develop\u00a0as a result of the medication,\u00a0which is being used.\u00a0That would be accountable for the\u00a0most significant\u00a0risk associated with stimulated\u202fIUI, which\u00a0is the risk of multiple\u00a0pregnancies.\u00a0\u00a0\u00a0<\/span><\/p>\n

            Therefore if\u00a0there are\u00a0several\u00a0mature follicles growing on the scan monitoring,\u00a0it\u00a0is\u00a0advisable\u00a0to abandon this\u00a0treatment\u00a0cycle.\u00a0<\/span>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”IUI Success Rates” tab_id=”1591171286847-d7d5abf2-7758″][vc_column_text]The IUI success rates vary depending on the woman\u2019s age at the time of treatment.\u00a0\u00a0\u00a0<\/span><\/p>\n

            The clinical pregnancy rate in women below the age of 35 is between 10-15% per attempt.\u00a0\u00a0<\/span><\/p>\n

            This\u00a0drops\u00a0to approximately 8-10% for women aged between\u00a035-40 and\u00a0it\u00a0further drops to below\u00a05%\u00a0in women above the age of 40.\u00a0<\/span><\/p>\n
